Plaque and tartar left on the teeth can lead to a variety of issues, as they are themselves a hotbed of thriving bacteria and other pathogens. These bacteria then cause gum inflammation which makes them more sensitive to bleeding and tearing issues. This bleeding and tearing is the first stage of the gum disease, called gingivitis. In this stage, the dentist may just perform scaling and polishing to get rid of the plaque and tartar and explain you the ways and methods to floss and brush your teeth in an more effective manner. But in case gingivitis is left without any treatment, it can become worse and develop into periodontitis, causing the formation of pockets full of pathogens which can cause significant tooth damage. Gum diseases break down the bone structure of the teeth making them loose. While this breakdown is irreversible, but gum damage can be treated with deep cleaning, which is also known as periodontal cleaning.

Dental deep cleaning, or periodontal cleaning is done in order to get rid of bacteria and other pathogens which thrive in hard to reach pockets. While scaling is just the removal of tartar from the surface of the teeth, root planing is the process where root surfaces are smoothened in order to clear those gum pockets. Generally both processes are performed in tandem for smoother and healthier teeth.

The dentist may numb the area to minimise pain for you by either using injectable anaesthetics or by using gel based anaesthetics in order to clean those gum pockets which are too deep. In some cases, though, it may cause little to no discomfort, especially when the pockets are shallow.

In order to ensure shorter times, the dentist may break up the cleaning process into parts or quarters, which are then spread over appointments in a number of days. The dentist would also use antibiotic gels and other medicines in order to completely get rid of bacteria and other pathogens.

Post the process, the teeth may become more sensitive to temperature changes. There may also be some bleeding as well as some discomfort might be felt. In these cases, brushing and flossing is to be done softly in order to prevent inflammation of the affected area and to give time for healing.

In case the pocket has been formed in an inaccessible location, the dentist may need to perform periodontal surgery in order to reach the inner pockets and perform cleaning. The surgery is done after giving anesthetic. This can be avoided if proper care is taken to clean the teeth properly as well as maintaining proper dental hygiene in general.
